Cancellation Policy

When you book an appointment with Dr. Terhakopian at his private office, time is dedicated to not only see you, but to prepare for your visit as well. Because we value your and Dr. Terhakopian's time, you will be expected to pay the full professional fee 48 hours before the day of your scheduled appointment. Cancellations made less than 24 hours before your appointment will incur a $50 fee. If you fail to present to your appointment, you forfeit your appointment and will not receive a refund. Both telephone and email are acceptable ways to alert us of a cancellation.

Additional Professional Services

We offer additional professional services including, but not limited to, report writing (e.g., disability reports), telephone conversations, writing support letters to other professionals/organizations (regarding short- or long-term work disability, emotional support/service animal recommendation, etc.), and preparing treatment summaries. Any of these additional services that take longer than 10-minutes are billed at $100 per 10-minute time increments unless a different amount has been agreed upon with our office.

Court proceedings and any forensic work (even if required to testify by another party) are billed at $800 per hour. Moreover, the hourly rate for any out-of-office proceedings are based on number of patient hours cancelled to provide this service.

Please Note: These additional professional services are for existing patients who have ongoing care at our practice only. These services are not offered as separate, one-time services.
