Initial Intake

Service Description

The Initial Intake Visit includes a 1.5 hour long appointment and includes extensive paperwork review by Dr. Terhakopian and an extensive face-to-face initial clinical interview with Dr. Terhakopian. Medications may be prescribed after visit if deemed medically necessary.

Cancellation/No-Show Policy

Cancellations made less than 24 hours before your appointment will incur a $50 fee. If you fail to show up to your appointment, you forfeit your appointment and will not receive a refund.  Both telephone and email are acceptable ways to alert us of a cancellation.
