New patients, before your initial intake appointment, it is required that you review and complete all of the following forms. These forms will be sent to you by one of our office's associates before your appointment. However, if you do not receive the forms, please contact our office immediately at 719-377-3993.

You may complete the following required forms by hand if you desire to do so. By clicking on the title of each form, it will download the form where you may print each form and complete them by hand. Please schedule a time to drop off your forms to us before your appointment by calling us at 719-377-3993, or you may fax them to us at 719-631-0655.

If these forms are not completed and sent to our office 24-hours before your appointment, your appointment is liable to be cancelled.
